Once we receive your paperwork we will contact you to schedule.

  • Prescription from pediatrician (may be needed for insurance – Typical Dx codes are: R13.11, R47.89, Q38.1 if needed)
  • Any pertinent paperwork (i.e. IEP, Evaluations, Swallow Study, etc.)
  • Cups and/or bottles that are regularly used with something to drink
  • A variety of food that the patient will eat crossing different textures such as a banana and pretzels (if age appropriate)

You can make a payment in several easy ways.  You can log into https://speechandmore.intakeq.com/, you can contact the office to charge the card on file or pay by cash or check.

If you are a private pay patient, you do not need a referral from a doctor. You will, however, need a prescription from your medical doctor or a doctor’s referral letter to make a claim through insurance.

Simply Communication, Ltd. is a preferred provider for most Blue Cross Blue Shield of Illinois PPO plans. While we are out of network with most other insurances, we will bill your insurance on your behalf.

We are proud to share that we also accept Care Credit.

  • Fill out necessary forms  
  • See your upcoming schedule

You may have received an email from the office that will link you to your portal.  If not, don’t worry! You can go to our website www.speechandmore.com and the bar across the top has a link to our “Patient Portal”. From here, click on the “don’t have an account” link on the bottom right of the box.  Please put in the PATIENT name and email address you believe we have on file. If you have the correct email address and patient name, you will receive an email with the password. If not, just reach out to us and we will be able to get things set up!

With the transition to the new portal (June 2024), the card on file was not able to transfer.  We do need every patient to log in and get a new card on file. If you are receiving invoice reminders, it is likely because we do not have a card on file.  You can log in to your account to check or contact the office and we can assist you.
